793 COLLATERAL HISTORY IN PATIENTS WITH CONFUSION

Abstract Introduction The aims of the project were to intervene early on admission, by taking a collateral history for patients with confusion; improve quality and detail being taken better establish certain clinically important specks information. These include triggers delirium, patient’s baseline cognition, any patterns in change cognition help determine if patient was displaying aspects type dementia prior mobility housing situation, continence, substance misuse alcohol etc. Ultimately, this identify next kin from which can be obtained, but also firm communication platform facilitate future discussions behalf undergoing delirium. This will ultimately, care making sure family members are involved exploring their ideas, concerns, expectations. Methods We created template booklet that introduced geriatric ward. Specific parameters set measured, pre-intervention post-intervention. A second round post-intervention data collection performed an improved sheet outlook. measured outcome 22 histories each round. Results found both rounds, post intervention, there compared almost all measured. For example, 100% documentation concentrations attention, issues orientation, expressive receptive dysphasia main concerns kin, problems executive functioning functional status patients. Conclusion history, overall quantity information gathered confusion and, contributed structured management.
